Broome Regional Aboriginal Medical Service (BRAMS) delivers a wide range of health and wellbeing services to more than 6500 Aboriginal and Torres Strait Islander people in the Broome region.

Our Aged and Disability Program is growing rapidly. Comprised of multidisciplinary teams which deliver community-based care, the service provides NDIS, TCP, Home Care Packages and psychosocial support – partnering with consumers, carers and familied and other service providers to deliver high-quality assessments, case management and therapeutic interventions.
About the Role
The Aged and Disability Case Manager is responsible for establishing a positive collaborative relationship with the person and their support network, and will assist them to identify, link with and coordinate supports which build skills, overcome barriers and achieve goals. The Case Manager is responsible for ensuring the implementation of a person’s plan and/or care package and the achievement of their goals.
Case management includes co-ordinating a variety of activities that support older people and those with disability to participate fully in community life, giving people more choice and control over how, when and where their supports are provided.
To be successful in this role, you will need:
- Demonstrated experience or understanding of the health and wellbeing issues affecting Aboriginal and Torres Strait Islander people;
- The ability to work with a broad range of people from a variety of backgrounds and experiences; and
- Passion about working with the local Aboriginal community to improve quality of life.
