Personalised Home Care for Elders

If you’re an older Aboriginal individual, BRAMS is here to support you with personalised Home Care Packages, ensuring you receive the care and assistance you require.

Based on the Australian Government’s assessment, your approved home care package will fall into one of the following levels, determined by your current needs:

Level 1

Basic care needs**

$10,271.10
per year

Level 2

Low care needs**

$18,063.85
per year

Level 3

Intermediate
care needs**

$39,310.50 per year

Level 4

High care needs**

$59,593.55
per year

At BRAMS, we offer a range of home care services that you can tailor to your preferences within the allocated package level.

Your Own Coordinator
When you join BRAMS, we understand the importance of personalised care. That’s why we assign you a dedicated coordinator to:

  • Work with you on your individual support plan
  • Collaborate with our health care professionals to meet your needs
  • Adjust your support plan if your needs change
  • Be your primary contact for any concerns or queries

Our coordinated approach to care ensures a seamless experience, bringing together a team of allied health professionals, including physiotherapists, occupational therapists, nurses, and speech pathologists.

Costs and Funding

We understand that costs are a concern. The Australian Government may expect you to contribute to the cost of your care with a basic daily fee and an income-tested care fee. BRAMS charges a care management fee and a package management fee, both tailored to your package level.

    Our engagement process is straightforward. Once you receive your approval letter, call our Intake and Assessment Team on 08 9194 4800. (We’ll guide you through the available services and set up a face-to-face meeting with our Engagement Coordinator to discuss your options and handle paperwork. A Client Coordinator will then visit you at home to develop a detailed care plan, ensuring you receive the care you need at your preferred times.
