Broome Regional Aboriginal Medical Service (BRAMS) is an Aboriginal Community Controlled Health Organisation that has supported the Broome community for over 40 years. Based in Broome, Western Australia, BRAMS delivers holistic and culturally safe primary health care, aged care, disability and community services to Aboriginal people across the Kimberley.

As an approved aged care provider, BRAMS delivers services under the Support at Home Program, the Commonwealth Home Support Programme (CHSP), and the Transition Care Programme (TCP). These services support older people to remain living safely and independently at home.

What You Will Be Doing

  • Delivering comprehensive clinical nursing care to aged care consumers in the community
  • Undertaking initial and ongoing clinical assessments
  • Monitoring chronic disease, wound care, medications, continence, falls risk, cognition and functional status
  • Providing post-acute and transitional care under TCP
  • Coordinating services under Support at Home and CHSP
  • Developing and reviewing care plans in partnership with consumers and families
  • Monitoring service delivery against approved funding
  • Escalating risks and clinical concerns in line with governance requirements
  • Working collaboratively with support workers, allied health professionals, GPs and hospitals
  • Maintaining accurate and audit-ready clinical documentation

This role focuses on continuity of care, coordination and safe clinical oversight in the community.

Is This You?

You are a Registered Nurse who values working closely with older people and supporting independence at home.

You will bring:

  • Current AHPRA registration as a Registered Nurse
  • Experience in aged care, community nursing or post-acute settings
  • Confidence undertaking comprehensive clinical assessments
  • Experience in care coordination or case management
  • Strong documentation and clinical governance awareness
  • Commitment to culturally safe practice when working with Aboriginal communities
  • A valid Australian driver’s licence
  • Experience in Support at Home, CHSP, TCP or within an Aboriginal Community Controlled Health Service will be highly regarded
