WE’RE HERE TO SUPPORT YOU
1 – Equivalent daily funding – Listed daily figures are indicative only. Support at Home classifications have an annual funding amount, and budgets are provided on a quarterly basis (annual amount divided by 4 quarters). Note that the listed daily figures are the sum of the base individual and base provider amounts in the Aged Care Rules 2025, where the base provider amount represents the funding deducted for the provision of care management.
2 – Restorative Care Pathway – Care Pathway provides episodes of non-ongoing targeted care services which can last up to 16 weeks. At times, participants with higher needs may require additional funding over the $6,000 allocated per episode. If approved, a participant can then access additional funding (up to $6,000) to provide a combined total of up to $12,000 for restorative care.
3 – The End-of-Life Pathway supports participants who have 3 months or less to live and wish to remain at home, by providing more funding to access in-home aged care services. A total of $25,000 is available per eligible participant over a 12-week period.
1 – Transitioned Home Care Package Recipient Level – Home Care Package (HCP) care recipients transitioning to the program will have a budget that matches the same funding level as their current HCP. They will also retain access to their HCP unspent funds and can use these for Support at Home services.

- Maintaining access and egress pathways through a property

- Weeding of established garden beds
- Yard clearance where there are issues of safety and access
- Essential pruning for access areas
Costs and Funding
We understand that costs are a concern. The Australian Government may expect you to contribute to the cost of your care with a basic daily fee and an income-tested care fee. BRAMS charges a care management fee and a package management fee, both tailored to your package level.

Our engagement process is straightforward. Once you receive your approval letter, call our Intake and Assessment Team on 08 9194 4800. (We’ll guide you through the available services and set up a face-to-face meeting with our Engagement Coordinator to discuss your options and handle paperwork. A Client Care Coordinator will then visit you at home to develop a detailed care plan, ensuring you receive the care you need at your preferred times.
